Common Misconceptions About "Prolonged Periods"

 

In recent clinical consultations, many patients have sought treatment for "prolonged periods"—where menstruation lasts for over 10 days, or stops for two to three days only to resume with continuous light bleeding.

Some also experience dizziness and fatigue but dismiss it as mere physical weakness or overwork, or even mistake it for the body undergoing a detoxification process. It must be clinically emphasized that an extended menstrual period is not a normal physiological phenomenon. Instead, it is a warning sign of a bodily regulatory imbalance, not a process of self-cleansing.

For most women, a normal menstrual period typically lasts between 3 to 7 days. If it exceeds 7 days, or if light bleeding persists for half a month without stopping, it is classified in TCM as an "extended menstrual period" (經期延長), and in severe cases, "flooding and trickling" (崩漏 - Beng Lou).

TCM believes that menstrual blood should "stop at a proper limit"; excessive volume or duration are pathological signs. Long-term neglect can easily lead to chronic blood loss, resulting in symptoms such as a sallow complexion, aversion to cold, palpitations, and poor concentration.


Medical Analysis: 3 Major Causes of Prolonged Menstruation

  • Common Western Medical Causes: These include uterine fibroids, endometriosis, endometrial polyps, hormonal imbalances, and medication factors (such as contraceptives or anticoagulants).
     
  • TCM Pathological Mechanisms (Categorized into 3 Types):
  1. Deficiency (虛 - Xu): Deficiency of Qi and blood compromises the stability of the Chong and Ren meridians, weakening the uterus's ability to contract and making it difficult for menstrual bleeding to stop.
    • Manifestations: Pale menstrual blood, fatigue, dizziness, palpitations, and aversion to cold.
       
  2. Heat (熱 - Re): Excessive heat in the blood accelerates its flow, causing menstrual blood to escape control and continuously overflow.
    • Manifestations: Bright red menstrual blood, dry mouth, irritability, and restless sleep.
       
  3. Stasis (瘀 - Yu): Blood stasis obstructs the uterus, disrupting the smooth flow of Qi and blood, preventing menstruation from ending according to its normal cycle.
    • Manifestations: Dark-colored menstrual blood with clots, intensified abdominal pain upon pressure, and recurrent light bleeding.

Principles of Treatment:

After ruling out organic lesions, treatment is administered based on syndrome differentiation according to the timing, volume, color, and texture of the menstrual blood. Clinically, a 3-stage approach is adopted: "Stemming the Flow (止血), Clearing the Source (調理體質), and Restoring the Old Way (恢復周期)."

The priority is to stop the bleeding, followed by regulating the body's constitution, and finally restoring the menstrual cycle—a process that typically requires 3 cycles of regulation.

This ensures that ovulation and endometrial functions stabilize; otherwise, the condition is highly prone to recurrence. It is common in clinical practice to see patients who only take hemostatic medications without regulating their constitution, leading to a monthly recurrence of the issue.


Prolonged Periods Hide Anemia: Hb < 12g/dL is a Warning Sign

During the acute bleeding phase, stopping the bleeding is the primary focus, which usually reduces the flow within a few days. However, stopping the bleeding does not mean a full cure; subsequent regulation over 2 to 3 cycles is necessary.

Prolonged periods are frequently accompanied by dizziness, palpitations, a pale complexion, and fatigue. In TCM, this falls under blood deficiency, whereas Western medicine attributes it mostly to anemia. Continuous blood loss leads to insufficient oxygen supply to the brain, causing temporary vision blackouts upon standing, as well as general weakness.If the following symptoms occur, it is recommended to test your hemoglobin levels.

A level of Hb < 12g/dL indicates anemia in women:

  • A noticeably accelerated heart rate after physical activity
  • Pale nails or lower eyelids
  • Frequent shortness of breath

Seek Immediate Medical Attention for Palpitations or Fainting

Do not wait if you experience any of the following; immediate evaluation is required:

  • Needing to change a sanitary pad every hour
  • Bleeding that lasts for more than two weeks
  • Severe palpitations or near-fainting spells
  • Known uterine fibroids or a possibility of pregnancy
  • Most of these cases constitute abnormal uterine bleeding and require prompt medical assessment.

Menstruation is the external manifestation of a woman's physiological rhythm, which should ideally be regular, moderate in volume, and self-limiting.
